Application Forms & Required Documents
- The applicant must fill and submit the online application. Alternatively you can fill out the Microsoft Document Application Form (TBD) and submit it to the address given below along with other required documents or fax the documents to the given number.
- The applicant must provide a full CV
- The applicant must submit an up to 3 pages describing his/her goals and motivations of participating in the workshop
- The applicants must submit an endorsement letter written and signed by their home department's chair indicating the benefit that the department will gain. The chair must indicate how the workshop will integrate the workshop materials into existing/new courses offered in their departments.
