Hazardous Waste Disposal Pickup Request Form

Please read and follow these instructions carefully -

Section-1                        

•  Select your Waste type     Chemical  Bio Waste

• Please fill out the form completely- incomplete forms will result in pickup delays.

• Room must be the location of the waste.

• Also a Transaction Number will be sent to you by email after you fill out the pick up request form. Please label all containers with this transaction number.

• Once the form submitted, the pick-up will be scheduled for Tuesday and Thursday accordingly.

• If you have any questions, please contact EH&S at 742-3876.    

• After fill out following form print it by using print button (below the form) or from your browser's file menu. Your page setup should be in Landscape.

Department	enter your department name
Room No	location of the waste
Email	enter your email (eg. Krish.Raya@TTU.EDU)
Type of Hazardous Waste	Chemical   BioHazardous check the appropriate type
Drum	Yes   No check the appropriate one
Is the request for Transfer of Chemicals and/or BioWaste form complete?	Yes   No check the appropriate one
Are all Hazardous containers labeled?	Yes   No check the appropriate one
