Hazardous Waste Disposal Pickup Request Form
.... Important Announcement ......
Due to the inclement weather all waste pickups scheduled for Tuesday Feb. 26 will be picked up on Thursday Feb. 28.

 

Please read and follow these instructions carefully -

Section-1                        

  • Select your Waste type     Chemical  Bio Waste Universal Waste

    Universal Waste: Paint, Flourescent Lights, Batteries, and Mercury Containing Equipment.

  • Please fill out the form completely- incomplete forms will result in pickup delays.

  • Room must be the location of the waste.

  • Also a Transaction Number will be sent to you by email after you fill out the pick up request form. Please label all containers with this transaction number.

  • Once the form submitted, the pick-up will be scheduled for Tuesday and Thursday accordingly.

  • If you have any questions, please contact EH&S at 742-3876.    

  • After fill out following form print it by using print button (below the form) or from your browser's file menu. Your page setup should be in Landscape.
