Please read and follow these instructions carefully -

Section-1                        

• Select your Waste type     Chemical  Bio Waste

• Please fill out the form completely- incomplete forms will result in pickup delays.

• Room must be the location of the waste.

• Also a Transaction Number will be sent to you by email after you fill out the pick up request form. Please label all containers with this transaction number.

• Once the form submitted, the pick-up will be scheduled for Tuesday and Thursday accordingly.

• After fill out following form print it by using print button (below the form) or from your browser's file menu. Your page setup should be in Landscape.
