ESC Checklist for New Employees
All new employees must complete the New Employee Packet within the first 3 days of employment.

E-Verify
- E-Verify (If applicable). If the position is funded with a federal contract (i.e., GSA, DOD, NASA, etc.), e-verify will be required. To document that the verification process has been completed, please enter the following on the top center of the I-9 form, "E-Verify Case Verification # _________" followed by the ESC's initials and the date. In addition, the employee's Tech ID (R#) should be recorded on the upper right hand corner of the I-9 form.

- Completed & signed
- Submit W-4 FORM ONLY. Worksheet & instructions maybe retained by employee or discarded.
Personal/Biographic Data Form
- Completed & signed
Employee Affidavit
- Completed & signed
Proof Of Selective Service Registration (If Applicable)
- Required to be completed by the employee
- Required for U.S. Citizens and permanent residents (Males 18-25 only)
- Screen-print an online verification from the Selective Service System web page.
- Your new employee may also register for Selective Service at this site.
Veteran's Status/Veteran's Employment Preference Form (If Applicable)

TRS Eligibility Identification Election Form
- Benefits eligible faculty and staff only. Students and other non-benefits eligible should not complete this form.
