A. I-9 New Employee Packet
Please complete your new employee packet and return the signed and dated forms to the Employment Services Coordinator in your department.
IMPORTANT NOTE: The new employee packet must be completed within the first three days of employment.

Texas Tech Employment Forms
Texas Tech employees enjoy a comprehensive choice of insurance plans including medical, dental, vision, life, accident, short-term disability, long-term disability, long-term care insurance, and TexFlex.
Full Time Employee (Faculty/Staff)
- The State of Texas / Texas Tech pays the health insurance premiums for full time employees and half the premiums for their dependents.
- If you are a full-time employee, working at a job where your regular schedule is 40 hours or more per week, you will be automatically enrolled in HealthSelect, if you make no health carrier selection, unless you formally decline health coverage.
Part Time Employee Or Graduate Student
- If you are regularly scheduled to work less than 40 hours or are a graduate student, you are considered a part-time employee.
- If you are a part-time employee, you may select health coverage for you and your dependents, but you will not be automatically enrolled. The State pays half the health insurance premiums for part-time employees and 25% of their dependents’ premium costs.
- If you have previously waived your coverage and your employment status has changed to Full-Time, please contact the benefits office immediately.

Complete the Employment Eligibility (I-9) Form. -
Step 2.
a. Complete the Initial Biographic Form.
b. Personal Bio Form - Update -
Step 3.
a. Read the Employee Conduct document.
b. Complete the Employee Affidavit Form. -
Step 4.
Proof of Selective Service Registration. (Male U.S. citizens 18-25 only) -
Step 5.
Complete the Employee's Withholding Allowance Certificate (W4) Form. -
Step 6.
Complete the Veteran Status/Veterans Employment Preference Form. (Optional) -
Step 7.
Complete the Crime Victim Identification Election Form. -
Step 8.

Step 9.
Complete the TRS-Eligibility Determination and Enrollment Authorization Form. (Benefits Eligible Faculty and Staff Only) -

Complete the Summary Notice of Privacy Practices Form (Benefits Eligible Employees Only) -
Step 11.
a. Benefits Orientation Form
b. Complete the Benefits Orientation Form for all new benefits eligible employees that become benefits eligible on or after 8/1/2009.

a. Any employee may set up direct deposit. If the employee would like to set up direct deposit, complete the direct deposit form below.
