CAMP APPLICATION HEALTH FORM SHUTTLE/EXTENDED STAY FORM CHECK OUT FORM - if your child is leaving camp at any point with someone other than their parent or guardian, we MUST have this form on file for you. We WILL NOT be able to accept verbal authorization. CAMP HANDBOOK - this has all of the camp information such as schedules, what to bring, etc.
