Assessment Stipend Request Forms:

The Office of Planning & Assessment asks that you provide detailed information about your funding request for the 2009-2010 academic year.  The travel description must specify how this activity contributes to the overall assessment plan within your Core area.  Your completed form is due to Lisa James in the Office of Planning & Assessment by 5:00PM, December 1, 2009.
