TTU Personnel - New Employee Packet

Welcome to Texas Tech! Please complete your new employee packet and return the signed and dated forms to the Employment Services Coordinator in your department.

The new employee packet must be completed within the first three days of employment.

		Please read all the following steps carefully. (Click on the appropriate step icon to go to the detailed instructions for that step).
		Each step will open in a separate pop-up window. If you are using pop-up blocking software, hold the control key down and click on the step icon.

Complete the Employment Eligibility (I-9) Form.

Complete the Biographic Data Form.

	a.		Read the Employee Conduct document.
	b.		Complete the Employee Affidavit Form.

	a.		Proof of Selective Service Registration.
			(Male U.S. citizens 18-25 only).

Complete the Employee's Withholding Allowance Certificate (W4) Form.

Complete the Veteran Status/Veterans Employment Preference Form. (optional).

Complete the Crime Victim Identification Election Form.

Read these important brochures:

-	Information for Injured Workers.
-	HIV and AIDS Brochure.
-	Sexual Harassment.
-	Campus Safety.
-	USERRA Notice of Rights.
-	Whistleblower Act.
-	Overtime and Pay Frequency Memo.

Complete the TRS-Eligibility Determination and Enrollment Authorization Form (Benefits Eligible Faculty and Staff Only)

Complete the Foreign Employee Information Form (Foreign Employees Only).

	a.		Forms Checklist
	b.		Submit completed packet to the department
