How to apply for a Social Security
Card |

These instructions apply if you have: |
|
|
- never been issued a Social Security Card.
Or
- need a replacement Social Security Card.
Or
- changed your name.
|
|
|
|
| Step 1 :
|
|
|
Complete an Application for Social Security Card. Click here for the Application, instructions, and a list of supporting documents you’ll need to take with you to the Social Security Office. |

| Step 2 :
|
|
|
Visit your local Social Security Office. Be sure to take your completed Application and supporting documents! |

| Step 3 :

After you receive your new Social Securiy Card, update your social security record with Texas Tech: |
|
-If you applied for a Social Security Card to process your Employment/Payroll records:
|
Provide a copy of your new card to the Employment Service Coordinator (ESC) in your department.:
Your ESC will submit the copy to Human Resource Services with the appropriate employment paperwork.

If you changed your name:
