Campus Climate & Incident Reporting Form

Texas Tech is committed to providing its students, faculty, and staff with an educational and workplace environment free from any form of unlawful discrimination. The Texas Tech community is dedicated to fostering and supporting a culture of mutual respect and communication.

Texas Tech University does not tolerate discrimination or harassment of students based on or related to sex, race, national origin, religion, age, disability, protected veteran status, or other protected categories, classes, or characteristics. While sexual orientation and gender identity are not protected categories under state or federal law, it is Texas Tech University policy not to discriminate for these reasons. Actions related to admission, discipline, housing, extracurricular and academic opportunities shall not be made based on a student's protected status. Discriminatory behavior is prohibited regardless of the manner in which it is exhibited, whether verbally, in writing, or electronically displayed or conveyed. Individuals who violate these policies and laws are subject to disciplinary action.

Making a Report

Several options are available for students, faculty, and staff to make a report to the university. The Campus Climate & Incident Reporting form serves as an additional resource for those who may be unsure where to best file a grievance, express concerns about campus climate and safety issues, or who may think their concern "doesn't fit" under another umbrella. These reports will be reviewed by RISE, and routed to other departments, such as a Title IX Investigator, Student Conduct, or the Dean of Students Office when requested or appropriate. This form can be filled out anonymously if you choose. You can use this report for incidences pertaining to yourself/your group (Student Org), a friend you are concerned about, or an incident you witnessed but may not know either party. Please note- should you choose not to provide identifying information, RISE and University staff will not be able to follow up with you about your report.

What does the Report Cover?

You can use this form to report incidences such as:

  • Bullying
  • Campus Environment
  • Domestic Violence
  • Hate Crimes
  • Hate Speech
  • Hazing
  • Micro-Aggression
  • Physical Assault (non-intimate relationship)
  • Sexual Assault
  • Sexual Harassment (including speech, actions, etc.)
  • Sexual Voyeurism
  • Stalking
  • Other

Why do we collect this information?

Texas Tech is committed to the safety of students, faculty, staff, and visitors. This includes not only physical safety, but emotional well-being. Gathering this information allows us to take a deeper look at our campus climate, and review areas for growth, education, and remedies.

What if I'd rather talk to someone in person?

If you would rather speak with someone face to face, the RISE staff can take reports either through appointment, Monday through Friday, 8:30 am to 4:30 pm or walk-in, M/W/F 3-5pm or T/TH 9-11am. RISE is located in the Student Wellness Center, Suite 204, directly across from the Student Counseling Center. Please be advised that under Title IX, Staff in the RISE office are "Jane/John Doe" reporters- meaning, we would be required to make a report if warranted (Title IX violations, Clery crimes), but can do so using pseudonyms and other non-identifying information.
