Group Travel Forms

The forms below are required for use of SGA allocated student organization funding. All are available in Microsoft Word format. Click to download the appropriate file and click "Save" to save a copy to your hard drive.

Group Travel Application

The Group Travel Application and Group Return from Travel Form should be used when an organization is sending more than one student (graduate or undergraduate) to a conference or competition. This form should also be used if only one undergraduate is traveling to a conference or competition. Please turn the Group Travel Application Form in at least 3 weeks before leaving on a trip.

Group Return from Travel

Please turn the Group Return from Travel Form in along with all original itemized receipts within 5 calendar days of returning from a trip.
