Medical Records Insurance Verification

An error in a Medical Information Bureau (MIB) file could cause insurance companies to rate a consumer less favorably. An insurance company that receives an MIB report will compare the report with the consumer’s information and errors will cause the insurance company to further investigate the consumer.

Because the report could reflect an error in medical records or reporting, you may want to request your MIB report by writing or phoning the following location:
