Investigator Financial Disclosure Requirements (Non-PHS funded)

All employees planning to act as investigators must disclose significant business or financial interests, including interests of their spouses, dependent relatives or household members, by October 1 of each year, and annually thereafter. Investigators should provide updates throughout the year as necessary to the Office of the Vice President for Research.

"Investigator" is defined as the PI, co-PI and any other person at Texas Tech University (TTU), TTU subgrantees, contractors or collaborators who are responsible for the design, conduct or reporting of research or educational activities that are funded or proposed for funding by an external entity.

How Do I Disclose My Financial interests?

All forms (those reporting no significant business or financial interests as well as those reporting significant business or financial interests) should be either completed in hard copy and forwarded to the associate vice president for research-research integrity using the instructions on the forms; or completed and submitted via the secure online questionnaire.

Hard Copy:

The forms are also available in hard copy for faculty who wish to use the current procedure. The PDF forms below have pre-loaded fields that can accept typed answers and electronic signatures.

  1. Form OP 70.37B – Confidential General Disclosure of Either Significant or No Significant Business or Financial Interests in Any External Entity
  2. Form OP 70.37C – Confidential Supplemental Detailed Disclosure of Significant Business or Financial Interest for Each Entity that would Reasonably Appear to be Affected by any Sponsored Project in Which you are Involved
