The Cayuse IRB submission will allow:
- Free text fields (using the proper formatting);
- Copy and paste functions into the free text field; and
- Ability to attach existing documents as a link or file to the appropriate section of the submission application.
Elements of Consent: Download

Some research proposal designs may be eligible for a waiver of written consent. Refer to the following forms to determine if your research project has a valid reason to request the waiver.

Attachment Examples (Recruiting):
What is said to human subjects for recruiting must be included in proposal packets. Modify the recruiting examples linked above to reflect the research study recruiting methods and procedures.
Claim for Exemption: Download
Required only for proposals submitted for exempt review. Check the appropriate boxes that relate to your specific study. Original signatures are required from all participating researchers. This is the second document the IRB Reviewer sees to grasp a context of the proposed study. The options on the form determine if the study can be exempt from federal regulations. Only the IRB can determine if the project is considered exempt.
Expedited Review Form: Download
Required only for proposals submitted for expedited review. Check the first box on the first page and any additional boxes that relate to your specific study. Original signatures are required from all participating researchers. This is the second document the IRB reviewer sees to grasp a context of the proposed study.
