TCVPA COVID-19 Testing Exemption Documentation Submission Form

The J.T. & Margaret Talkington College of Visual & Performing Arts (TCVPA) has implemented a COVID-19 testing program for participants in specific performance-based activities and groups. Pursuant to Governor's Executive Order GA 39, the University cannot compel you to receive a COVID-19 vaccine or require you to disclose whether or not you have received the COVID-19 vaccine to receive services from the University or enter any University property. 

However, if you voluntarily provide documentation disclosing that you have received the COVID-19 vaccine or have tested positive within the past ninety (90) days, you will be exempt from the TCVPA COVID-19 testing program.  Providing proof of exemption vaccination is not connected in any way to any academic evaluation and will serve only to exempt you, pending review of your submitted documentation, from the TCVPA COVID-19 testing program required for participation in performance-based activities and groups. Your voluntary  disclosure of this information to the University is not a condition of receiving any service from the University or entering into any University property.

YOU ARE NOT REQUIRED TO DISCLOSE WHETHER OR NOT YOU HAVE RECEIVED THE COVID-19 VACCINE TO TEXAS TECH UNIVERSITY.  ANY DISCLOSURE OF YOUR COVID-19 VACCINATION STATUS IS VOLUNTARY.
