If you have already been accepted into the Department of Computer Science, fill out this form to apply for financial support. Financial support is awarded on a competitive basis and preference is given to Ph.D. students and to Masters students that choose the 30−hour thesis option.
- Statement of Purpose
Fill out this form when applying to the Department of Computer Science. You should detail your reasons for applying for graduate study as well as your academic and professional interests and goals.
- Letter of Recommendation
Fill out this form when applying to the Department of Computer Science. Have at least three letters of recommendation sent to the Computer Science department from those familiar with your educational and/or work background.

- Leveling Requirements
If you have already been accepted into the Department of Computer Science and you do not have an undergraduate degree in Computer Science or Computer Engineering, you must complete leveling courses. Fill this form out with the graduate advisor to determine the necessary classes to take.
- CS 5000 Approval
If you want to take CS 5000 during an internship and/or need a letter generated for Curricular Practical Training, please fill out this form and submit to the graduate advisor. You will need a letter from your place of employment describing your work.
- CS 7000 Approval
If you want to count a CS 7000 course towards your degree plan you must get approval from both the graduate advisor and your academic advisor.
- Course Registration
Fill out this form to register for courses. This form must be used during advanced registration.
- Request for a Memo
Fill out this form when you need a memo on letterhead from a faculty or staff member.

If you need to change your graduation date, add or remove courses, add or remove committee members, or change your thesis title, you must submit this form for approval.
