Faculty, Students, Staff and Visitors
This form is for faculty, staff, students, and visitors to the university that have encountered an incident that needs to be reported and could possibly involve a violation of the Texas Tech Code of Student Conduct. Once submitted, this form is able to be read by those being accused or any other parties directly involved in the incident. If you have questions as to whether or not you should submit this form due to an incident you observed or were involved in, call our office. If you would like to see the various Code of Student Conduct violations applied to Texas Tech Students, click the link to the left. This is a new form that is submitted on-line. Please mail all supporting documents to: Student Judicial Programs, Box 45029, Lubbock, TX 79409 or Through campus mail MSC 5029.
This form should be used by: Texas Tech Faculty, Staff, Students, and Visitors.
Residence Life Staff should use the Residence Life reporting process to document incidents and NOT this form.
Faculty and All Staff should speak with their supervisor concerning the details of the incident being reported in addition to filing this form with Student Judicial Programs.

This form allows students to grant access to their educational records at Texas Tech to other individuals (parents, attorneys, etc.). FERPA stands for the Family Educational Rights and Privacy Act of 1974. Students
Students sign this form when attending their designated disciplinary hearing. This forms gives students the opportunity to understand their rights and responsibilities when proceeding through the university disciplinary process.
Students utilizing this form gives Student Judicial Programs authorization to verify medical treatment, evaluation, and/or substance use education from a Texas Tech or community health provider.
Students considering going overseas to pursue academic goals as a Texas Tech student should read up on this brochure. Going overseas is a great opportunity to represent the United States, the state of Texas and Texas Tech! Learn how to stay safe and best represent what it means to be a Red Raider.
Authorized judicial staff in Student Judicial Programs and Residence Life should use this form to refer students to the Raider Assistance Program for concerns with substance use, including alcohol/narcotics.
Those students needing to take Alcohol EDU may use this instruction sheet to login and take the course.
Faculty
This brochure was developed for Texas Tech Faculty by our office to assist in academic dishonesty issues and to handle troublesome behavior with students in the classroom. Tips include recognizing improper behavior, suggestions for making the syllabus clear with classroom and professor expectations, and steps to follow with the incident reporting process.
Feel free to use this sample letter in your correspondence with the student. This will help educate the student on what you have witnessed and what action you plan to take.
