Access to Mental Health Records

Clients who wish to view their mental health records may do so by contacting the SCC front office and completing a Release of Information form.  A staff member will review the records with the client and provide either a hard copy or an emailed PDF within 14 days.  Due to limitations of our electronic record-keeping software, we are unable to provide immediate access to records through an electronic portal.
