*In order to conduct safe and effective laboratory activities, teachers must follow proper lab safety procedures with their students while using STEM CORE traveling labs. As a teacher checking out this lab, I accept responsibility for keeping students safe, utilizing PPE as needed, and disposing of chemical waste in proper containers. I also agree to return all items clean and accounted for, and store chemicals properly for transport. Any damage to items will be reported to STEM CORE in a timely manner.

*All STEM CORE Traveling Labs and equipment are offered to teachers at no cost to their schools. In an effort to continue to offer these resources at no cost, we require teachers utilizing them to complete a follow up survey and provide pictures of their students (FERPA approved) utilizing these labs/equipment.

I will complete the required teacher feedback survey in a timely manner and share pictures of students approved for posting utilizing the labs/equipment.
