Application Checklist
Before submitting your PREP-Lubbock application, review each requirement carefully. Applications must be complete before they can be considered.
Student Application Checklist
Before You Submit
- Receive permission from a parent or guardian.
- Read the entire application carefully.
- Answer all questions on the application.
- Complete the required essay.
- Include a recent grade report.
- Make a copy of the completed application for your records.
Teacher or Counselor Nominations
Give the two nomination forms included with the application to your teachers or counselor. One nomination must be from your mathematics teacher. The second may be from your English teacher, science teacher, or counselor.
Teachers or counselors should submit nomination forms directly to PREP-Lubbock in school envelopes.
A Complete Application Includes
- Completed application form
- Required essay
- Copy of the student’s latest report card
- Two nomination forms submitted by teachers or a counselor
Important: A student’s application will not be considered for acceptance unless all required materials are completed and submitted.
