Technology Access Requests for New Hires

Enter the new hire's eRaider username. Leave blank if eRaider is not active.
Enter the customer's phone number. If the customer cannot be reached, please include the requestor's preferred number.
Enter the customer's TTU email address or other email if TTU account is not active yet.
Enter the new hire's position title or role in the college. (Ex: administrative assistant or professor of practice).
Enter the associated department, center or requesting unit name.
Enter the customer's assigned office number (and cubicle if applicable).
